This position is primarily responsible for identifying, coordinating and performing intake and continuity of care services for persons with Intellectual and Developmental Disabilities. This position will also assist with managing the IDD General Revenue and HCS interest Lists. Employee in this position will be required to interact with individuals, families, community agencies and service providers. Work will include educating others about IDD services at the Center, services and supports provided through HHSC, coordinating and facilitating eligibility determinations, assisting in coordination of placement into waiver programs, ICF and State Supported Living Center Facilities. Duties will also include support tasks for the IDD Intake and Continuity of Care, including data entry into both CARE and Anasazi data systems, checking and facilitating Medicaid eligibility, general filing and clerical support duties. Must be self-directed and articulate, demonstrative of good judgment and comfortable working with minimal oversight.
Requires at least a high school diploma/GED. Prefer 2-5 years of proven experience providing administrative support. Requires above average computer skills and ability to use common office software (Word, Excel, etc…) Prefer some experience in a social service field. Must possess a current Texas Driver’s License and qualify for Center insurance coverage.
