DESCRIPTION

This position reports to the MH Program Administrator with clinical/nursing supervision/oversight by the Nursing Supervisor. This position is a PRN/temporary nurse role for the ACT Team and is responsible for the coordination/oversight/implementation of physician's plan of care pursuant to the weekly medication clinic for the ACT Team clients with the Center psychiatrist. This position is responsible for coordinating services with other Center staff, outside agencies, and civil authorities to provide services for individuals on the ACT Team and for individuals in Level of Care 3. This position is responsible for assisting in maintaining an up-to-date community resource network. This position will be available via phone for consultation with Case Managers regarding medical/nursing issues for ACT patients. Services will be provided in the office, community, and at other sites in the Center catchment area (Taylor, Jones, Callahan, Stephens, and Shackelford Counties). Services and documentation of those services will follow all state and federal standards, including Medicaid requirements. This position will demonstrate positive and professional interaction skills with all consumers, family, staff, and outside agencies. This position must consistently meet monthly performance targets.

This position provides a variable level of support to consumers with mental health issues in an integrated Psychosocial Rehab treatment team structure. This includes but is not limited to providing assistance in accessing medical, social, educational, or other appropriate support services as well as linkage to more intensive services if needed, in addition to frequent monitoring, assessment of service needs, service planning and coordination, as well as providing Psychosocial Rehabilitative services within Level of Care 4. These duties are performed in accordance with established policies, procedures, and ACT "Best Practice" guidelines, with some latitude for personal judgement.

This position may be required to cover responsibilities in other departments to include Adult Mental Health and the medication clinic.

Hours and duration of this position will be determined based on center needs on an ongoing basis.

QUALIFICATIONS

• Registered Nurse License. Psychiatric certification preferred.
• Experience in mental health/intellectual and developmental disabilities preferred.
• Must have a vaild Texas Driver's License and be an insurable driver.
